Documents required from you:

  2. Current Passport
  3. Foreign nationality acquisition date
  4. Birth Certificate
  5. Marriage Certificate (If any)
  6. Spouse Passport
  7. Latest educational qualification
  8. Visible Body Mark (If any – Short description)
  9. Parent’s name (as per passport)
  10. Current address
  11. Emergency contact
    1. Name
    2. Address
    3. Mobile number
  12. One passport size photo (Not older than 6 months) – White Background
  13. Naturalization certificate

If you have your most recent passport and naturalized before June 1st, 2010:

  1. Two passport size photo (Both digital and physical copy of the same photo) (Not older than 6 months) – White Background
  2. Proof of address document
    1. Driving license / utility bill / lease agreement
  3. Photocopy of the information page and the last two amendment pages of the current passport
  4. Did you last-held Indian passport expired at the time of naturalization?
    1. If no
      1. No documents required
    2. If yes
      1. Affidavit of Naturalization (Original & Notarized)
    3. Most recent Indian passport
      1. Photocopy of 1st three page
      2. Photocopy of last 2 page
    4. Naturalization Certificate

 

If you don’t have your most recent Indian passport and naturalized before June 1st,2010 :

  1. Two passport size photo (Both digital and physical copy of the same photo) (Not older than 6 months) – White Background
  2. Declaration of renunciation of citizenship of India form
  3. Proof of address document
    1. Driving license / utility bill / lease agreement
  4. Photocopy of the information page and the last two amendment pages of the current passport
  5. Did you last-held Indian passport expired at the time of naturalization?
    1. If no
      1. No documents required
    2. If yes
      1. Affidavit of Naturalization (Original & Notarized)
    3. Sworn Affidavit
    4. Naturalization Certificate
    5. Proof of Indian Origin (Provide Any 2 Documents From Below)
      1. Copy of The Election Card/ Voter Id
      2. Copy of Board Certificate (Ssc) Or Higher Level
      3. Copy of Birth Certificate
      4. Copy of Parent’s Proof of Indian Origin (First 3 Pages And Last 2 Pages Of Parent’s Indian Passport Copy /Parents Birth Certificate)
    6. Applicants Naturalized After 2010 Provide A Police Report In Case An Indian Passport Is Missing.
    7. Copy of Green Card
